1. Print out order form 

           a. email completed form to [email protected]

           b. turn it in either at equipment handouts or at the first few practices

2. Complete form in person at equipment handouts or at the first few practices

1. If turning in completed form in person you can either pay by check, exact change, credit card, or request an invoice to be sent.

2.  If emailing in completed form an invoice will be sent to the email address listed on the order from once the order date of August 11th closes.
